Please print, sign (wet signature) and scan Form 52303 and combine it with all the other documents required as indicated on the form into one single PDF attachment. The file size must not exceed 2 MB.
Please name your file as follows: Surname, First name_request.pdf
To send your request to the EPO, go to the contact form and select/upload your file. You will receive an enquiry number by email. Where necessary, e.g. if the attachment exceeds the maximum size, you can use the contact form to send a second file to complete your request. In this case, please make sure to indicate the enquiry number you received.
